Fountain Medical Centre is currently accepting new patients. To register you must live within the practice boundary (click here for a map).
Simply call into the practice and complete a registration form, and hand to reception with proof of your address. For example:
- Utility Bill
- Bank or Building Society Statement
- Driving Licence
- Offical Letter e.g. from a Solicitor, Accountant etc
Only recent and original documents will be accepted.
The Receptionist will then arrange for you to attend a New Patient Check appointment with a nurse.
Once the form and the health check are completed you will be fully registered with the practice and free to see any of our doctors.
