The Memory Support Worker Service

The Memory Support Worker service was commissioned by Leeds Clinical Commissioning Groups and is delivered by a partnership between the Alzheimer’s Society and Leeds and York Partnership (NHS) Foundation Trust.

Memory Support Workers (MSW’s) work closely with local GP practice teams, the Memory Service team including occupational therapist, consultant psychiatrist, nurses and the Younger People with Dementia Team.  We receive referrals from the Memory Service and other agencies but we can also receive self-referrals.

The criteria for the role of the MSW is:

  • to enable people to access the memory service in order to receive a diagnosis
  • to provide information and advice to both the person with dementia and their families and friends
  • to enable people living with dementia to participate in community and social life
  • to enable people living with dementia to access appropriate support in a timely manner.
